Change the way the .PDF file is generated to use XSLT and FO, instead of using jade and DSSSL.

This fixes bug #1432655; logohelp.pdf has strange indentation of examples

Make it so that the HHC doesn't show the root. Now, each chapter is shown at the top level. In addition to saving the user a click, the TOC also auto-updates to whatever the user clicks on in the index. This is a major improvement.

Remove extraneous whitespace from within <userinput> tags. I also fixed a few typos that I noticed along the way.

Extraneous whitespace became important when I started using CSS for the HTML output. Now that I shade examples in grey, any extraneous newlines is very apparent (and looks amateur-ish).

Add a way to generate HTML that is suitable for uploading to the FMSLogo homepage. I also added style sheets to the HTMLHelp content .HHP for the OSs that support it.

Add a benchmark test. I'm not sure where I'm going with this, but it's nice to have *some* benchmarks, at least.

Add regression tests for FOREVER.

Fix typo

Dead Code/Dead Store removal:

Remove some legacy functions and global variables that delt with manipulating the terminal. FMSLogo will always have it's own GUI, so terminal capabilities doesn't make sense.

This change makes the code simpler and smaller, and possibly faster.

Refactoring: Rename val_status to g_ValueStatus and create manifest constants for the possible values.

The names that I chose for the values aren't that great, but it will be easy to replace them, now that they are no longer magic numbers.

Add a nascent CSS for styling the HTML generated from logohelp.xml.
