this happens me too. 
approx 2 weeks ago I questioned. no reacti0n.

from log_
2009-01-17 14:31:06,651 fail2ban.actions.action: ERROR  iptables -D
INPUT -p tcp --dport ssh -j fail2ban-SSH
iptables -F fail2ban-SSH
iptables -X fail2ban-SSH returned 100                                  
                                      

please, what happens there?
can anybody tells more...
