I would recommend formatting a fresh floppy and using cat/dd as before. (or it could be a dirty/old/broken floppy drive. As a test, you could make a .iso file and burn it to a CD to see if that makes any difference. I'm pretty sure it's the floppy or the drive, and not the image though.
