But AGPMode 4 (in Device section) is a typical candidate for instabilities. Try commenting it out or setting it to a lower value (2 or 1). Also the AGPSize 128 is usually not necessary. It refers to main memory which can be used for textures. Unless you have extreme demands the on-card memory + default AGPSize will be more than sufficient.
