#199 fix for unlock speed + cycles=max/auto

With CPU_CycleAutoAdjust, unlock speed does not work properly.
Some people in my DOSBox Korean forum complained that in protected mode games, for example, the speed does not become faster though they pressed Alt-F12.
It is significant because they needed to quickly pass some tedious opening scenes by hitting the hotkey.

This patch includes:
1. fix for unlock speed in cycles=max/auto
2. Ctrl-Alt-F12 for a continuous turbo mode
