as a notice to book writers, maybe the latex backend is good enough.
i am willing to give support for real problems, meaning not the pathologic
test.txt. sorry my time is limited and there are so much possibilities.

so if someone wants a pdf from his rest-docs contact me.

maybe we schould split test.txt, so one could get a compliance page,
which construct works how good with which writer.

ad ToC: maybe have a look into latex style:
  toc = table of contents, lot = list of tables, lof = list of figures
  and when generating pdf these are bookmarks (i guess).
