just to add:

I have the following test.rst :

list1
-----

list2
-----

list3
-----

when I perform a C-c C-t C-t, a new buffer opens "Table of Contents" that
displays the 'headers'
list1
list2
list3

so I assume the index function works, but there is no way to create ir on
the top of the screen
(cursor on the top left corner + C-c C-T TAB)
