The cell containing the entrytbl (nested-table) didn't render at all - in fact "c2" appeared in cell b2 and cell c2 was empty. Incidentally I've just read the limitations page of FOP and regarding tables it says: "There are two limitations for tables: 1) FOP needs you to explicitly specify column widths 2) Cells have to contain block-level FOs. They can't contain straight character data.". I'm not sure whether this is related or not.
