For an example, see the attached document, a simplified version of linux kernel documentation. (DTD in this sample is V3.1; tests with V4.1 give the same result) This is not caused by docbook-utils modifications to stylesheets: I tried the example with and without the db2ps -d option to bypass these modifications, and found no difference. This occurs in docbook-style-dsssl-1.76 (as distributed in RedHat Skipjack beta 2), Layout was still correct in 1.64 (as found in RedHat 7.2).
