Current linkback from an index entry is to the
parent element (e.g. chapter) of the indexterm.

Request that (if present) the indexterm id value
be used as the href target.

This would match with fo, page number as the target
and would be smarter than the parent chapter level
target.

I'll leave it to the WG to decide if indexterm
is changed to require the id value.

I added a new param index.links.to.section, set to 1 by default to retain the current behavior. When set to zero, the index entry links to the precise location of the anchor generated for the indexterm, instead of linking to the section title.
