#106 crosslinking with <programlistingco/>

if you are using the <programlisting/> element
in conjunction with the a <calloutlist/> element, it is
possible to link the co and its describing callout
arearef element. that way you can cross link between
callout bugs and their description.

in my opinion if someone uses the <programlistingco/>
element he/she should get exactly the same behaviour,
because the <programlistingco/> is just a container
element for a programlisting and a calloutlist.

actually, the <programlistingco/> does not support
linking or even cross linking.
