#13 glossary item appear toplevel navigation

all glossary divisions appear top - level in the
navigation of a generated pdf document

docbook-dsssl-1.76/1.77 (works correct in 1.73 though)

(only the top glossary should appear in the navigation,
with the divisions and item as subs)

googled around and some time ago have found s.b.
posting a solution for it... it seemed that simply some
specification how to handle the glossary has been
forgotten in the print/docbook.dsl
(sorry cannot find it now :-()
