#1343 Duplicate xmlns:d declaration in xsl-ns/template/titlepage.xsl

http://snapshots.docbook.org/xsl-ns/template/titlepage.xsl declares xmlns:d twice.

I found this when trying to build docbkx-tools with the snapshot XSL.

This was an issue with the snapshot builds, not 1.78.1. I fixed it in the snapshot builds and for future releases.
