#4 Remove list from hi and emph

In the current 2.1 DTD, the <emph> and <hi> elements
can contain <list> elements. That is contrary to how
those elements are defined, as they are supposed to
mark “words or phrases”. These two elements exist to
draw attention to words or brief phrases…not to bold or
italicize entire sections of text. The two elements
should be modified so they cannot contain lists.

Including <list> as a sub-element of these two tags was
an error in the first place, and is contradictory to
their intended use.
