ddclient just pipes emails to the "sendmail" command, so you'll have to configure your sendmail client to use your preferred server. There are plenty of lightweight MTAs that would probably suit you, my go-to choice for lightweight is ssmtp but I have not tried to use it with an Exchange server.

You can set up an SMTP relay on the machine running ddclient. I believe that's the way it was designed to work.
