#46 cntlm service should start without credentials

At my workplace the IT uses a proxy that does not need any credentials (uses MAC or IP for authentication). However CNTLM fails to start if the username, domain and password is not configured.
Our proxy server ignores any credentials so I could set some dummy values for all the three, but in general CNTLM should start without credentials, along with the warning messages in the system log.
