I've compiled and can run pocketsphinx_continous on win64, and I'd like to
test with microphone, but I have no idea what to put as the device name. I get
a FATAL_ERROR at the end, "line 250: Failed to calibrate voice activity
detection"
I searched the forums and google and the docs but I can't seem to find the
answer. Is there someone out there who knows?

For MSVS windows build device name doesn't matter. You need to adjust your
volume settings to make sure it actually records speech. You need to check if
your recordings have DC offset. Zero recording level and DC offset are a
common reasons of failed calibration.
