In doc/cl-ent.xml:

<!ENTITY cpp ...

For &cpp; IMO you could refer at the GNU C preprocessor's manual at
http://gcc.gnu.org/onlinedocs/gcc-4.0.0/cpp/. It is one of the few good
manuals of the GNU project.

I'd rather not refer to versioned pages.
OTOH, it would be nice if the C FAQ referred to it...

You could point to the HEAD version: <http://gcc.gnu.org/onlinedocs/cpp/>
This URL should remain constant.
