Multiple bugs in Makefiles

There are several problems with a few of the Makefiles included in BLT. These problems include:
* Omission of '-p' flag to mkdir causes blt to fail to create directories in certain environments
* $(INSTALL_ROOT) missing from some installation commands; $(INSTALL_DIR) used in a few places where $(INSTALL_ROOT) should be used instead.
* BLT_LIBRARY not used consistently as the script installation directory
* libdir hardcoded to ${exec_prefix}/lib in ./configure
* Installion directory for headers can't be set using the --includedir configure switch
* Two man pages conflict with identically named files from tcllib and tk (graph.n and bitmap.n)
The attached patch fixes all of the above issues.
