#28 A to Z Item provider permission check

Adds a permission check option to the content item A
to Z provider - if selected, then only pages that the
current user has access to are displayed.

This functionality of course requires that the a to z
pages are not cached.

nb - upgrade scripts are marked as 1.0.2 to 1.0.3 - this
will need to be amended.
