#10 Forms not getting transformed when selected as index items

When a form is assigned as an 'index item' and then
viewed in a web browser, the form is not being
displayed. The XML contains all of the relevant
information but it is not being transformed.

The problem still exists in a new IBM theme and so must
be an issue with the 'behind the scenes' XSL.

Just thought I'd better document this!
