Use $spam_quarantine_to to divert the mail to an IMAP folder. Combined with 
procmail we manually review all mails with a medium spamscore and any false 
positive is moved to another IMAP folder on the server. From there a cron job 
makes sure to run sa-learn and redeliver it to the X-Envelope-To recipient.
