When a query clears and re-populates a select element with options, the blank answer with the label "-- please select --" is not readded.

I fixed this bug while fixing another AJAX query bug (see bug [1701652] Changing Values for Cascading query results in invalid data).

This bug passed system testing using the Option Queries test script in STS005-Wizard Form.
