#330 Create PSI Error

Steps to recreate:

1) Login to AW and create a new form
2) Select a Form Template and a Form Name and click Continue
3) An error will follow

When I looked into this, you seemed to be able to create a PSI if you first went into the Admin tab and managed a form a template. I also am having trouble to get it to re-create now, so it may have just been a server glitch -- we should still test to make sure.
