After installing OOo 2.3.1, KeePass 1.10 no longer started with OnScreenKeyboard 1.10 and, after logging in to KeePass, when "Open URL(s)" was clcked, started Mozilla Firefox 2.0.0.12 rather than IE 7.0.5013.13.
Firefox is the default browser but KeePass has to start IE so it can AutoType username and password (IE has the necessary API and Firefox does not -- yet).
In KeePass, Tools->Plugins showed OnScreenKeyboard (1.10) disabled. After re-enabling it, exiting KeePass, starting Keepass, OSK appeared but "Open URL(s)" still started Firefox.
I can't think of any other changes, apart from OOo 2.3.1 installation, that happened between KeePass working normally and these changed behaviours.

I am 100% certain I did not remove that manually! It's not as if something reverted keepass.ini to default because the Database.kdb file is in a non-standard directory and that's still OK.
In KeePass options, "Use puTTy for ssh: and telenet URLs" was no longer ticked.
The main issue is not how to fix the configuration but that it changed unepectedly. OK -- I probably did something stupid or the computer "glitched" (changing configuration for a plugin, the .ini file and options!).
The reason for posting is in case something systematic is going on here; if it happens to other KeePass installations and people search this forum, ...

Searching all the filesystems also showed a shortcut to C:\Program Files\KeePass\KeePass.ini in the Recent folder and two shortcuts to it under the History folder.

Sounds like KeeForm is not set to be the default URL handler in KeePass.

Spot on. keepass.ini no longer contained this line:
KeeUrlOverride=cmd://"{APPDIR}\KeeForm.exe" "{URL}" "{USERNAME}" "{PASSWORD}" {ENTERFORM}

Do you have more than one KeePass.ini? Search all of C:.
