The jquery plugin is not working. The online example demonstrates this (screen shot included).
I tried figuring out why, and I noticed that the tinymce jquery plugin script does not contain something resembling the jquery plugin code "jQuery.fn.foobar = function() {

I just installed a clean copy of 1.10.10 after running into this problem, but it remains: no popup for a password is shown and the error is "access denied".
After some clicking around it turned out that setting the "progress messages" to "loud" is a work-around; the popup is shown in that case.

I installed the test version mentioned in the 2008-08-20 20:09 post.
It seems the problem is gone, but I get the impression it is replaced with TortoiseCVS crashing. This crashing happens not always, but in about half of the cases, that is why I suspect the crashing replaced the doesn't-work. After a crash I can simply retry and then it works. I must mention that I use Vista64, so that may...

As in "not at all"? I mean; it is sporadic, but as still as frequent as "one out of two", so frequent enough.
I could see if I can setup a VMWare instance where it occurs. Then you could use that and see if it also occurs on your system. This would eliminate all factors except geography...

I experience the same problems using 1.10.4. There is an "unnatural" amount of access denied errors. This co-incidentally is a clean install of Windows with a freshly accepted key.
Funny thing is, on the same machine I run a VMWare development environment which still has a 1.8 version. Doing an update on the 1.10 may result in several access denied messages, while at the same time updating...
