That error is almost certainly because you installed libconcord into /usr/local (/usr/local/lib/libconcord.so) and your system isn't configured to look for libraries there.
You can:
1. set an LD_LIBRARY_PATH environment variable
2. Change /etc/ld.so.conf
3. Install into /usr (/usr/lib/libconcord.so)

Hey. Sorry for the delay in response, I'm not sure why I didn't get emails on this bug. Strange.
A few comments:
1. I think the patch's uninstall hook... attempts to install it again. But my autoconf-fu is bad, so I could be wrong
2. I'd like you to add some notes to the README about this in your patch before I accept it.
