You can create the top level categories, but you do not have to add the whole list while applying the keywords. Just do not use the "join" mode but the "last" mode instead. It is easily switchable on top of the keywords dialogue list. (See http://mapivi.sourceforge.net/097-Keywords.png or http://mapivi.sourceforge.net/mapivi10-keywords.png on the left if you're using current dev trunk.)

As far as I'm informed keyword tags in iptc must not exceed 64chars. (See e.g. http://www.controlledvocabulary.com/imagedatabases/iptc_naa.html#IPTCchart)
They easily reached if I add a whole chain of keywords. This however, is desireable, indeed, to allow for a hierarchical search like "give me all photos of cities" and then drill it down to whatever.
How is this handled in...

As mapivi needs to have a jpg one might consider using dcraw to get such a jpg from the raw for preview (a comment in mapivi code refers to nefextract but that would only work for Nikon; dcraw handles a lot more formats). According to the man-page of dcraw:
