[FIX] forums: Remove dependency on the tiki_p_forum_lock permission on the admin forums page since admins will by definition have this permission and since tiki_p_forum_lock has not been implemented the lock/unlock actions will otherwise never be shown. Note the TODO on the view forum thread page that the dependency on tiki_p_forum_lock will cause these actions to not show on that page.

[FIX] actions: only duplicate this column if it's the last column in a table. Fixes tiki-list_comments.php, for example, where the action column is already the second column and therefore doesn't need to be duplicated as the first column.

[REF] ajax services: Simplify confirm utilities so that json encoding/decoding is not necessary, allowing for easier implementation for other action confirmations
