Using the reference SIEMENS mpeg multiplexer with a
toolame encoded file at 16 KHz / 16Kbps fails.
(tried only on Win32, don't know if it's a multiplexer
problem).
Does not happen with 32 KHz / 32 Kbps.
1) Encoding:
toolame.exe -s 16 -m m -b 16 test16.pcm test16.mpa
2) Multiplex:
mpeg2s test16.mpa test100cif.mpv final.mpg

A few people have reported that every now and then toolame
will encode totally stupid frames (which then show up as
noise on playback). Haven't tracked it down.
