#2 win32 and setup.py patch

The C code now uses strtok_r, which isn't available in the native mode of the GNU compilers. The
patch works around this by adding the necessary file in the windows/ directory. It also fixes other
minor things in the setup.py file (the new URL for pyPgSQL).

win32 binary release for version 1.4.
