Add ability to formConsult (aka formLetterhead) for printing directly to a proprietary clinic letterhead.
formConsult Action Class
User will select a Letterhead form from the Patient Encounter Forms library.
User may like the current format of the formConsult and will have the option not to use a proprietary clinic letterhead.
Users whom are currently unhappy with the printed format of the current formConsult will not benefit from the enhancement.
Self testing in a pilot clinic.
No requirements. The process will be intuitive.

Where will the proprietary letterhead be stored? Is the proprietary letterhead just a PDF that will be written on, Can anyone create these PDFs? Can it handle more than one Proprietary letterhead?
In the eForm image directory.
Proprietary = letterhead that is designed and created by the clinic, rather than the generic inserts that Oscar uses.
PDF Letterhead = writable PDF document
Yes, anyone can create these PDF’s.
Users can also choose not to use the feature.
Does a clinic really have more than one letterhead? How does Oscar currently handle more than one clinic letterhead?

I know the consult request has a letterhead dropdown..usually clinics have only 1, but it seems that doctors would like to use their own at times.

Just to be clear. I am referring to the “formConsult" letterhead not the letterhead generated from Oscar's Consultation module.
I do not see a drop down for multiple letterheads in the “formConsult” letterhead - but perhaps it is only active while multiple letterheads exists?
Adopters in the community will have two options:
1.) adapt this code in the future to allow multiple “Proprietary Letterheads” - the code will remain as extensible as possible.
or
2.) turn off the feature to use “Proprietary Letterheads” and remain using the current option that allows multiple letterheads.
