After upgrading to mod_ntlm 0.4 I get the following in the
error log:
got header with host "T", domain "A"
NTLM/SMB user "i": authentication failure
for "/ntlm/orlof.php4"
And a prompt for username and password. After typing
in the username and password I get the following in the
error log:
got header with host "THOR-306B-2",
domain "ALTHINGI"
NTLM/SMB user: "ALTHINGI\ingvi": authentication OK
Any thoughts?

If you're using ColdFusion, try upgrading to the latest version.
Afaik ColdFusion 4.5 somehow strips the NTLM Header.
