Need to add a --scratch mode that works only with script mode.

What it would do is simply assume that the database does not even have the migration_info and migration_info_audit tables, does not prompt for connection url information, and simply always assumes that you are starting from version 0 and moves onward.

This would be better referred to as "offline" mode. Scratch mode is very confusing.
