#88 Improve action mapping documentation in xsetwacom man page

Some improvements to the documentation for mapping button actions that I was missing, from reading the source and verified by experiment.

• xsetwacom(1) "Button":
  • note what happens when buttons are released
  • describe what "modetoggle" does
  • general notes about how keys are specified
  • note limit on number of actions
• wacom(4): note that only simple button presses can be configured in xorg.conf
