#4 missing field in questions table

when trying to ask a question there is an error:
****************DB ERROR - invalid query

that could be because in the query in insert.php,
there is a field called submittedby that isn't
specified in the questions table.

also, answeredby is not in the questions table. I added both
of these fields as varchar (32).
