Clicking "Add hyperlink to menu item" pops up a "Select a menu item" window; however, some +/- icons on the hierarchy list are missing. Please check the attached screenshot on which I put red rectangles to locate the missings.

    It is not the missing of +/- icons, but the line linking sublists is broken; so it needs to click the broken point to unfold or fold sublists.

    fixed in next preview. The menu structure contained some pseudo entries that had to be omitted from the dialog
