#4 GrandTec Grand X Guard support

Add support for the GrandTec Grand X Guard card.
This comes in 2 types, 4-port and 16-port. I have a
4-port card, but it seems this is actually the 16-port
card with edge connectors not soldered on to the card.
All 16 ports on my 4-port card seem to work fine, so I
assume the 16-port card is identical and the patch
should work with it. Tested only on Solaris x86.
