#12 Charset missing in XHTML output

I guess you should have the utility include some char-set info in the code of XHTML1.1 output. This issue causes a very "garbled" output when I use the last ramp (the one at the bottom: █▓▒░ ). It turns OK when I switch to UTF-8 on browser. Hereby I think the following extra line should appear in the XHTML code:

    Browsers are supposed to assume that the encoding is UTF-8 or UTF-16 if it isn't set, so I've never encountered that problem. But you're right, I probably should be specifying the encoding so I'll add it in the next version.
