I have been unable to get help on https://community.open-emr.org I used to use this Sourceforge forum and some helpful person always managed to help me to a solution. I complete the 'Add Patient' form and click 'Create New Patient'. Nothing happens. I am using OpenEMR 5 (just upgraded from version 4.2.2) with the latest patch applied, on Lubuntu 14.04.5 LTS. I'd appreciate some help. Many thanks.

I notice that my OpenEMR does not remember and display patients' telephone numbers...

Got it. Yes, that allowed the upgrade to proceed and I can now log into "italy" successfully....

Sorry to be dense, but what do you mean by "<browser_address>" in "<browser_address>sql_upgrade.php?site=italy"....
