I was analyzin the debug log again and found this to be salient "Clip copy to fast difference from last copy". Searching here for that I found this reply. I could not find the setting "ignore copies that occur X milliseconds from the last copy", but the "Save Clipboard Delay" looked close enough. I had already used the GUI to set it to zero before and it did not help. I decided to check the settings file directly and found the line with "SaveClipDelay". The value was set to 1000. If I copy multiple different strings without much delay between each copy event, Ditto won't add some of them to the database. That delay is usually the time it takes for me to process a line of text, push a button and for the copy code to execute. Haven't timed it, but I would guess half a second or less. As far as I can tell, it is a timing issue. Keeping the Ditto window open to see when it fails to add a clip to the database, I immediately tried to paste when I saw it failed and the pasted clip...
