Yes, it must be mutool, it shouldn't be used unattended. I have been using mutool for a long time now and I've had several PDFs with the same issue and I have reported some bugs but still, missing content is, in my experience, somewhat common unless the PDF file is only text and no images. You should definitely check every PDF that gets optimized.

I have no AV installed. Maybe they are referring to these attributes: https://docs.microsoft.com/en-us/windows/win32/fileio/file-attribute-constants Instead of the ones you get with GetFileAttributesEx
I've tested this with jpg files and only the modified times are being preserved, not only for the processed files but for the backups too. The other times (accessed and created) are being set to the current date. Version: 15.90.2730 (x64)
