Kind of answering my own question here, I added a windows rule which I only found out is possible, for Class "ApplicationFrameWindow" to automatically close these windows, so far no more ghost windows.

I've done a bit more digging on this using the virtual win logging, some interesting log lines. Hi, I'm using the very latest Windows 10 release but I've been seeing this problem for a long time on previous windows versions. I'm using the latest 4.5 release. I use a couple of plugins and both show the same problem, which is, application windows that have no names / titles and don't exist as a application that can be interacted with. The only way for them to not show up in the inconized plugin is to set them to 'always show'. I run virtualwin as administrator as well, if I restart explorer.exe...
