read-only fields - configurable

another feature to add would be to have read-only fields in phenote.

this was something that arose during the BIRN conversation, but might be useful for many groups, that might import data from external databases or applications.

this should be configurable in the config. i wonder if it should be configurable, but possible to change on-the-fly? like enable/disable fields if necessary?
