Add IVTV support to livepause

Here is the patch that I sent John and Duncan in August, as there has now been some interest on the mailing list for this feature I decided to upload it untested (no IVTV card here) it shouldn't break DVB support but may not work for IVTV.
Also required for this to work is MPEG-PS-to-TS from (http://www.charrett.plus.com/mpegpstots.tar.bz2).

Download the above tar file, unpack, configure, make and install it.

To use in freevo simply remove other TV player plugins and activate tv.livepause and set you IVTV videogroup type to 'ivtv' ie group_type='ivtv'.
