#72 Subscribe page needs Full Name blank

The Subscribe webpage needs a blank for Full Name to go along
with Email Address.

This full name should show up along with the other info in the
Admin page when you go to approve a subscribe request.

This allows the list owner to screen subscribe requests to closed
lists, as it's rare these days to be able to tell who the requester is
from their email address.
