About

Clinical Extract pulls research data from any EHR with a certified bulk FHIR API: Epic, Oracle Health (Cerner), athenahealth, eClinicalWorks, MEDITECH, NextGen and Veradigm. It runs inside your environment as a SMART Backend Services client with read-only scopes, exports only the study cohort through a Group $export, and turns the NDJSON into one analysis-ready CSV with a data dictionary that names the FHIR element behind every column. Patient data never leaves your environment. It is built for cancer and clinical registries (coded case and abstraction fields in the NAACCR layout), research informatics teams and honest brokers (study-scoped pulls with an IRB-ready data dictionary), and research sites (clinical trial feasibility counts from coded criteria). Features include a cohort builder, roster pull with FHIR $match, the CSV and data dictionary, and an oncology lens that delivers biomarkers and TNM stage as named columns.

​FHIR is described as a 'RESTful' specification based on common industry-level use of the term REST. In practice, FHIR only supports Level 2 of the REST Maturity Model as part of the core specification, though full Level 3 conformance is possible through the use of extensions. Because FHIR is a standard, it relies on the standardization of resource structures and interfaces. This may be considered a violation of REST principles but is key to ensuring consistent interoperability across diverse systems. For each "resource type," the same set of interactions is defined, which can be used to manage the resources in a highly granular fashion. Applications claiming conformance to this framework claim to be conformant to "RESTful FHIR." Note that in this RESTful framework, transactions are performed directly on the server resource using an HTTP request/response.
