WheelFromHere is a free, open-source middle click wheel scrolling application.
For applications without scrolling support, WheelFromHere can middle click on anywhere and simulate mouse wheel up and down depending on the current mouse cursor position relative to the original middle click position.
The wheel scrolling indicator is shown to the bottom right of the mouse cursor. (See screenshots)
