SudokuDesigner.exe 
can be run under Windows simply by clicking it. 
It needs a .NET Framework 3.5 being installed.
Linux user can use Mono.

SudokuDesignerInstallV01R04.zip
is the Windows-Installer, who creates
a startmenu-entry and a desktop icon too. First
you have to extract the files. (Use e.g. 7zip, 
WinZip.) Try this version if the way above doesn't 
work.

SudokuVDesignerSourceV01R04.zip
is the compressed folder of the project. 
The sources are written in Visual C# 2010.
