Fixed
- Start with Windows now reflects the exact Windows login item and its enabled or disabled StartupApproved state.
- Duplicate legacy startup entries are consolidated into one canonical registration without changing the user's choice.
- The settings toggle now reports when Windows rejects a startup change.
Validation: 128 automated tests passed, plus a successful Windows NSIS release build.
